Fees and Payment
Managed Care (HMO, PPO, POS, etc.) patients must present their plan identification cards when registering for each office visit. Co-payments, deductibles and charges for non-covered services must be paid at each visit.We ask that you pay $50 toward your deductible at the time of service versus the entire amount until your deductible has been met.We will bill you for the balance after insurance responds.
For patients not covered by insurance in which PAMPA participates, we require complete payment for our services. We will call with a complete total for services within 48-72 hours after your visit. Vaccines must be paid for at the time of service. Reimbursement may be obtained from your insurer. At the time of your office visit, please let the staff know you will need a detailed statement of the charges incurred. Your charge slip is accepted by most insurance companies as the "physician's statement" when you submit your claim to them. We understand that this may be difficult for families at times; therefore, our patient account representatives will be happy to discuss payment arrangements with you.
We accept cash, personal check, Mastercard, Visa, Discover and American Express.
Administrative Service Fee
In order to cover services such as vaccine records, long distance calls, school excuses, F.M.L.A. forms, insurance applications, computer generated account reports, completion of camp, school and physical forms, etc., our practice finds it necessary to charge an annual Administrative Service Fee of $25 per patient. This annual fee is intended to cover the cost of certain administrative services we may provide which are not covered by your insurance.
Missed Appointment Fee
Broken appointments represent a cost to us, to you, and to other patients who could have been seen in the time set aside for you. We reserve the right to charge a fee for canceled or missed appointments. This fee is higher during school holiday weeks. We request 24 hours notice for cancellation of appointments.
A fee will be charged for a missed appointment. The third consecutive missed appointment may result in discharge from the practice.
